Attach to pages when the user has broken any ToS/Policys. If the user has a Level 2 warning already the use the Template:UW3
Usage:

1 - Page name. Leave blank if N/A - You will only have to type the page name E.G: Scott or if a user then User:Scott.
2 - Who reverted the edit. If its you then add ~~~ and it will automatically show your name with a link to your page.
3 - Reason. Leave blank if N/A
4 - Any added punishment - if left blank it will show as None.
5 - Warning added by - add ~~~~ if its yourself. If left blank it will show as N/A.
